1. Notify the professional office staff at O.P. ALLEN FUNERAL DIRECTORS.
2. Call your loved one’s private doctor if he/she was ill and seeing a private doctor.
3. Call the nearest police station. Shortly after the call is placed, police officers will visit the house of mourning. If your loved one was ill and seeing a doctor at the health centre please inform the officers at this time. After investigating, the police officers will advise you accordingly on the following step.
4. Find and keep in your possession your loved one’s identification card and electronic birth certificate (Application for Birth Certificate). These items are needed to register the death.
5. Collect the Medical Cause of Death from your private doctor or the district Medical Officer (DMO). This document together with your loved one’s birth certificate and ID are needed to register the death.
6. Register the death at your district registrar. (Registrar Listing) From the registrar you will receive 3 temporary death certificates (one is needed for the church, one for the funeral home and one for the cemetery or the crematorium) and 1 registration form.
7. Go to Ministry of Legal Affairs to apply and collect the electronic death certificate. To apply for the electronic death certificate you will need the registration form you received from the registrar. (Application for Death Certificate)
